HOW TO DO IT
A product add-ons or custom fields plugin. These add fields to the product page and carry the customer's input through to the order.
Choose one that shows the input clearly on the order in admin and in the order emails, or you will be hunting for it when packing.

That the field is required where it must be That the input appears in the admin order view That it appears in your order confirmation email That it survives to the packing slip if you print one

Some plugins let an option add to the price. Useful for engraving charges and upgrades.

Keep the number of fields small. Each one is a decision the customer must make before buying, and every decision loses some of them.
