WHAT TO ASK FOR
The email address. That is all you need.
Every additional field loses subscribers. A name is sometimes worth it if you will use it; anything more rarely is.
WHAT TO SAY
What they will receive, and how often.
"Monthly. Practical guidance and occasional offers. Unsubscribe any time."
That sentence does more than any design.
WHAT NOT TO DO
Pre-tick the consent box. That is not consent. Bury the subscription in terms nobody reads. Use a pop-up that appears immediately, before the visitor knows what you do.
ON POP-UPS
They gain subscribers and lose visitors. Measure both rather than assuming.
Delaying one, or triggering it on exit, is usually a better trade than showing it on arrival.
AFTER SUBMISSION
Confirm clearly. A form that appears to do nothing gets submitted repeatedly.
